Today, I looked down to see the time, and I noticed that the 7 O'clock stick marker was missing. Upon shaking the watch, I saw it loose inside the case, up against the side. I immediately stopped the movement by pulling the crown to prevent damage to the movement. Looking specifically at the OP's watch, the marker does not appear to be rotated, but rather shifted about 0.02mm to the left (measured off of the OP's picture, rescaled to 100 pixels per mm). I've owned many a Rolex rep over the years of many types and qualities from all our dealers. The one consistant problem that send them to the watchmaker is the markers falling off the face. Does this EVER happen with Gens? You have two choices, 1) send to Rolex service center. They will inspect it, if it needs a service, they may also require a new dial. If everything is genuine Rolex, the cost will be 0.00 or more. 2) find a competent watchmaker, they can re-cement the index marker back on. The markers usually come off from a drop or another impact.
